Cardiogeriatric: A 21st Century Perspe

Cardiogeriatrics in the 21st century has become a central discipline in the face of accelerated aging in Latin America. This article reviews the historical background, demographic transition, and clinical challenges posed by multimorbidity, frailty, polypharmacy, and healthcare inequities. It examines the evidence gap due to the underrepresentation of older adults in clinical trials and highlights the importance of integrating biological age, functional status, and cognition into risk stratification. Expanded prevention strategies are emphasized—including adapted Life’s Essential 8, vaccination, and social determinants of health—along with the partial reversibility of frailty through tele and rehabilitation and gerontology. Finally, the GOAL algorithm is presented as a practical framework for patient-centered decision making in older adults with cardiovascular disease. In cardiogeriatrics, in prevention is the future.
